Step 2: You will receive an email within one business day with available dates and times. Reply to the email with your preferred date and time for an appointment.
Step 3: Look out for your appointment confirmation email. The email will include how-to instructions to complete your intake forms. The paperwork must be submitted before your initial appointment.
